Job Description

Make a difference in patient care by providing home health risk assessments to Medicare Advantage members in Delta County. This per diem opportunity offers flexibility and autonomy, allowing you to create your own schedule and work shifts that fit your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling options
  • Set your own minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct comprehensive home health risk assessments in members homes, typically lasting 45 minutes per visit
  • Review and evaluate medical history, chronic conditions, and current medication use
  • Educate patients on health management and medication adherence
  • Identify gaps in care and recommend appropriate next steps to ensure optimal health outcomes
  • Focus exclusively on assessmentsno responsibility for treatment planning or prescribing

Qualifications:

  • Active Nurse Practitioner license issued in Colorado
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum of 6 months recent NP experience preferred

You will visit patients in Delta, Orchard City, Paonia, and surrounding areas within Delta County, so reliable transportation is required. For each evaluation, compensation ranges from $80-$90, with an additional mileage stipend and pay for administrative time associated with each visit.
