Step into a rewarding per diem opportunity providing home health assessments for Medicare Advantage members in Oklahoma County. As a Nurse Practitioner specializing in home health, you will connect with patients in Oklahoma City, Edmond, Midwest City, and nearby areas, delivering compassionate, high-quality care where it matters most.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Maintain a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate patients current and prior medical history, conditions, and medications
- Offer thorough education regarding diagnoses and medication management
- Detect gaps in care and offer actionable recommendations
- Visits typically take about 45 minuteseach may be an annual assessment or post-discharge check-in
- Note: This role does not require you to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- At least six months of recent Nurse Practitioner experience preferred
- MSN from an accredited nursing program
