Job Description

This per diem opportunity enables you to provide vital care to Medicare Advantage plan members across Greene County, INincluding Stockton Township, Linton, Bloomfield, and Jasonville. Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingdesign your own work hours
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct 45-minute home health risk assessments for Medicare Advantage plan members
  • Complete in-home annual health assessments and post-discharge visits as assigned
  • Review prior and current medical history, including medication regimen and conditions
  • Educate patients on medical diagnoses and medications to promote health literacy
  • Identify and communicate gaps in care; offer recommendations for follow-up
  • No requirement for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner (NP) license in Indiana
  • Current BLS/CPR certification
  • Board Certified through AANP or ANCC
  • Minimum of 6 months current experience as a Nurse Practitioner (preferred)
  • Master of Science in Nursing (MSN) from an accredited institution
