Seeking a dedicated Nurse Practitioner with experience in home health to perform risk assessments for Medicare Advantage members throughout Lake County, FL. This per diem opportunity offers autonomy and flexibility, making it ideal for those seeking balance in their work schedule. Youll play a vital part in preventive health, helping patients in cities like Clermont, Leesburg, and Eustis stay on a path to wellness.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments (averaging 45 minutes per visit)
- Review and document medical history, current conditions, and medication information
- Educate patients on health topics and safe medication use during in-home visits
- Identify care gaps and recommend potential resolutions
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active registration as a Nurse Practitioner in Florida
- Current BLS certification
- National board certification through AANP or ANCC
- MSN degree from an accredited institution
- Minimum six months of recent experience as an NP, preferably in home health
- Excellent interpersonal and assessment skills
