Make a significant impact in community health as a Nurse Practitioner specializing in home health assessments for Medicare Advantage plan members in Sumter County, including The Villages, Wildwood, and Bushnell. This per diem role offers flexible scheduling, allowing you to design your own work week and practice at locations throughout the county based on shift needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to match your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patients residences
- Review and evaluate patient medical history, existing health conditions, and medication regimens
- Deliver impactful education to patients on medical conditions and proper medication usage
- Detect care gaps and provide actionable recommendations to resolve them
- No treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner (NP) License in the state of Florida
- Current Basic Life Support (BLS/CPR) certification
- Board Certification from AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
