Deliver high-quality care and utilize your clinical expertise as a Nurse Practitioner delivering in-home health risk assessments. This per diem opportunity offers you the autonomy to set your own schedule while serving Medicare Advantage plan members across Marion County. You may conduct visits in communities such as Ocala, Belleview, or Dunnellon, providing essential services that contribute to optimal health outcomes for clients post-hospitalization or as part of their annual health assessments.
Schedule:
- Flexible shifts available Monday through Sunday

- Options for weekly or bi-weekly minimum hour commitments
Responsibilities:
- Complete comprehensive annual and post-discharge health risk assessments in member homes
- Review individual medical and medication histories to evaluate and document current conditions
- Educate patients on their diagnoses, care plans, and medication usage
- Identify gaps in care and recommend solutions to enhance wellness and disease management
- Administrative time included; no treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Florida
- Current BLS/CPR certification
- Board Certification as an NP (AANP or ANCC)
- MSN from an accredited institution
- At least 6 months of current NP experience is preferred
- Background in homehealth and risk assessment is valuable
