Experienced Nurse Practitioners are needed for per diem home health risk assessment opportunities in the Oak Ridge area. This role involves conducting comprehensive health evaluations within patients' homes throughout Knox County, including Farragut, Powell, and Galesburg. You will play an important part in supporting Medicare Advantage plan members through detailed assessments and patient education, ensuring quality, patient-centered care.
Schedule:
- Flexible shift options available Monday through Sunday
- Design your own schedule to fit personal and professional commitments
- Choose your preferred weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments and post-discharge visits as scheduled
- Evaluate patients current and historical medical conditions, including a thorough review of medication history
- Offer education on conditions and medications to help patients make informed health decisions
- Identify gaps in care and make recommendations to improve health outcomes
- Note: This role does not require you to prescribe medications or develop long-term treatment plans
Qualifications:
- Active Nurse Practitioner license in Tennessee
- CPR (BLS) certification is required
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN from an accredited nursing program
