Seeking a Nurse Practitioner to perform in-depth home health risk assessments for Medicare Advantage plan members across Knox County. In this per diem position, you'll play a vital role in providing holistic care, ensuring member well-being, and supporting preventative health strategies.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to design your own work schedule
• Minimum weekly or bi-weekly hourly commitments necessary

Key Responsibilities:

• Complete comprehensive annual health risk assessments in home settings for Medicare Advantage plan members.
• Review prior and current medical histories, assess medical conditions, and evaluate medication use.
• Educate patients on their health conditions and proper medication management.
• Identify care gaps and make recommendations to address them.
• Perform assessments such as annual wellness checks or post-discharge visits; treatment planning and medication prescribing are not required.

Qualifications:

• Active Nurse Practitioner (NP) license in Tennessee.
• Board Certification by AANP or ANCC.
• Current BLS certification (CPR).
• Graduate of an accredited MSN nursing program.
• At least six months of NP experience preferred, but not mandatory.
