Deliver high-quality care in a per diem, flexible role as a Nurse Practitioner specializing in home health risk assessments. This position focuses on conducting thorough evaluations for Medicare Advantage members in Oak Hill and throughout Fayette County, including Fayetteville and Montgomery. Schedule:

• Flexible shifts available Monday through Sunday
• Set your own working hours
• Schedules determined weekly or bi-weekly as preferred

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for individuals on a Medicare Advantage health plan
• Review prior and current medical histories, assess ongoing medical conditions, and evaluate medication use
• Educate patients about their health conditions and medications to support informed decision-making
• Identify gaps in care and offer recommendations for follow-up, improving patient outcomes
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in West Virginia
• Board Certified via AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR certification (BLS required)
• At least six months of current Nurse Practitioner experience (preferred)

This opportunity offers compensation of $80$90 per completed evaluation, with additional mileage stipend and payment for administrative time.
