Job Description
Take on a meaningful role in home health by performing comprehensive risk assessments for Medicare Advantage plan members across Scioto County. This per diem opportunity offers Nurse Practitioners the chance to utilize their clinical expertise through in-home visits, where youll conduct detailed evaluations in locations such as Portsmouth, Wheelersburg, New Boston, and Minford.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for a Medicare Advantage health plan, spending approximately 45 minutes per evaluation
- Assess prior and current medical history, existing conditions, and medication regimen
- Deliver direct patient education regarding medications and ongoing conditions
- Identify care gaps and recommend solutions to support optimum health outcomes
- Visits may include annual assessments or post-discharge evaluations
- No requirements for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Completion of MSN from an accredited school of nursing
- At least 6 months of recent Nurse Practitioner experience preferred
This position offers compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and paid administrative time.
