Exceptional opportunity for a skilled Nurse Practitioner to deliver home health risk assessments on a per diem basis throughout St. Clair County, Illinois. This role offers a high level of scheduling flexibility and serves Medicare Advantage plan members, including those in Belleville, O'Fallon, and East St. Louis.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Self-scheduling to meet your availability
- Minimum weekly or bi-weekly hour commitments
Key Responsibilities:
- Perform comprehensive annual or post-discharge home health risk assessments during 45-minute visits
- Collect, evaluate, and document prior and current medical histories, conditions, and medication use
- Educate patients on their medical conditions and medications to support ongoing care
- Identify gaps in care and make recommendations to improve patient outcomes
- No requirement to prescribe medications or create treatment plans
Qualifications:
- Active Nurse Practitioner license in Illinois
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months of recent NP experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
