Job Description

Exceptional opportunity for a skilled Nurse Practitioner to deliver home health risk assessments on a per diem basis throughout St. Clair County, Illinois. This role offers a high level of scheduling flexibility and serves Medicare Advantage plan members, including those in Belleville, O'Fallon, and East St. Louis.

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Self-scheduling to meet your availability
  • Minimum weekly or bi-weekly hour commitments

Key Responsibilities:

  • Perform comprehensive annual or post-discharge home health risk assessments during 45-minute visits
  • Collect, evaluate, and document prior and current medical histories, conditions, and medication use
  • Educate patients on their medical conditions and medications to support ongoing care
  • Identify gaps in care and make recommendations to improve patient outcomes
  • No requirement to prescribe medications or create treatment plans

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of 6 months of recent NP experience is preferred
  • Master of Science in Nursing (MSN) from an accredited institution
