Experienced Nurse Practitioners are needed to provide home health risk assessments for Medicare Advantage plan members in Madison County, Illinois. This per diem position allows for flexible scheduling throughout the week. Practitioners may be assigned to various locations such as Edwardsville, Alton, Granite City, and nearby areas during their shifts.
Schedule:
- Flexible shifts, Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patients homes
- Review and evaluate patient medical histories, current conditions, and medication usage
- Deliver education to patients regarding their medical conditions and proper use of medications
- Detect care gaps and offer recommendations for improved patient health outcomes
- Visits may qualify as annual assessments or post-discharge follow-ups
- No prescribing or creation of treatment plans required; focus is on evaluation and education
Qualifications:
- Active Nurse Practitioner license in Illinois
- Current CPR (BLS) certification
- Board certified via AANP or ANCC
- At least 6 months of recent NP experience (preferred)
- Master of Science in Nursing (MSN) from an accredited program
This opportunity offers compensation of $80$90 per evaluation, plus a mileage stipend and additional reimbursement for administrative time. You'll contribute to improving health outcomes for Medicare Advantage members while maintaining autonomy over your work schedule.
