Job Description

Step into a valued per diem opportunity as a Nurse Practitioner, focusing on home health risk assessments within Harford County. This position allows for flexible scheduling, enabling you to create shifts that best fit your lifestyle. As part of this role, you'll conduct evaluations in locations such as Aberdeen, Bel Air, and Edgewood. Each assessment takes approximately 45 minutes and may include annual check-ups or post-discharge visits for Medicare Advantage members.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Review and document prior and current medical history, including medication usage and ongoing medical conditions
  • Educate patients on diagnoses and prescribed medications
  • Identify care gaps and provide actionable recommendations to address them
  • No prescribing of medications or development of treatment plans is required

Qualifications:

  • Active Nurse Practitioner License in Maryland
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months of recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Compensation includes $80$90 per evaluation, with additional mileage reimbursement and administrative time provided.
