Experienced Nurse Practitioners are needed for per diem home health risk assessments in St. Lawrence County, NY. This flexible opportunity allows you to create your own schedule, with shifts available any day of the week. As a provider in this role, you will conduct in-home evaluations of Medicare Advantage plan members throughout locations such as Norwood, Potsdam, Massena, Ogdensburg, and Canton.
Schedule:
- Flexible shifts Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments, each visit averaging about 45 minutes
- Assess prior and current medical history, chronic conditions, and medication use
- Educate patients regarding their health status and medications
- Identify care gaps and provide actionable recommendations to optimize wellness
- Visits may include annual assessments or post-discharge follow-ups
- No responsibility for treatment planning or medication prescribing
Qualifications:
- Valid Nurse Practitioner license in New York
- Board Certification (AANP or ANCC)
- MSN degree from an accredited institution
- Current BLS (CPR) certification
- At least six months of recent Nurse Practitioner experience preferred
