Opportunity available for Nurse Practitioners interested in home health risk assessments in the St. Lawrence County area. This per diem position offers flexible scheduling and the chance to make a meaningful impact on patient care for Medicare Advantage plan members. Assignments may take place in various communities within the county, offering variety with each shift.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling optionscontrol your own calendar
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform comprehensive annual home health risk assessments in patient homes
- Review medical histories, current conditions, and medication use
- Educate patients on medication management and chronic disease awareness
- Identify potential gaps in patient care and recommend next steps to improve health outcomes
- No prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in New York State
- Master's Degree in Nursing from an accredited program (MSN)
- Board certification by AANP or ANCC
- Current BLS certification required
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation for this position ranges from $80 to $90 per completed evaluation. Additional stipends are provided for mileage and necessary administrative time.
