Embrace a dynamic opportunity delivering home health risk assessments for Medicare Advantage plan members throughout Windham County, CT. This per diem position allows you to make a meaningful impact while enjoying scheduling flexibility and autonomy. Providers will travel to member homes in areas such as Windham (Willimantic), Killingly, and Mansfield, completing comprehensive assessments with a patient-centered focus.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members in the patient's home environment
- Review and evaluate current and prior medical history, conditions, and medications
- Educate members on their medical conditions and treatment regimens
- Identify care gaps and provide recommendations for improved outcomes
- Focus on assessmentthere is no requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Connecticut
- Current BLS CPR certification
- Board Certification through AANP or ANCC
- Minimum of six months recent NP experience preferred
- MSN from an accredited nursing program
