Job Description

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate medical histories, current conditions, and medication regimens during 45-minute visits
  • Educate patients about their medical conditions and medications
  • Detect gaps in care and recommend solutions to address them
  • No treatment plan development or prescribing medications is required

Qualifications:

  • Current Nurse Practitioner license in Ohio
  • Board certification via AANP or ANCC
  • Basic Life Support (BLS/CPR) certification required
  • MSN degree from an accredited nursing program
  • At least 6 months of NP experience preferred
