Step into an impactful role supporting community health as a Nurse Practitioner focused on home health risk assessments in Huron County, OH, including Norwalk, Willard, and Bellevue. This per diem opportunity offers remarkable flexibility, allowing you to customize your work schedule to fit your lifestyle, with shifts available any day of the week.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate medical histories, current conditions, and medication regimens during 45-minute visits
- Educate patients about their medical conditions and medications
- Detect gaps in care and recommend solutions to address them
- No treatment plan development or prescribing medications is required
Qualifications:
- Current Nurse Practitioner license in Ohio
- Board certification via AANP or ANCC
- Basic Life Support (BLS/CPR) certification required
- MSN degree from an accredited nursing program
- At least 6 months of NP experience preferred
