Enjoy the flexibility of per diem work as a skilled Nurse Practitioner dedicated to delivering high-quality home health risk assessments in Fairfield County, CT. Working with Medicare Advantage members, your visits will focus on comprehensive risk evaluations, which may include annual assessments or post-discharge check-ups. Each session, averaging approximately 45 minutes, empowers you to make a direct impact in the communities of Bridgeport, Stamford, and Norwalk.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform in-home risk assessments for Medicare Advantage plan members
- Review and document medical history, current medical issues, and medication use
- Educate patients regarding health conditions and prescribed medications
- Identify gaps in care and provide actionable recommendations to promote better patient outcomes
- No medication prescribing or creation of treatment plans is required
Qualifications:
- Valid Nurse Practitioner license in Connecticut (CT)
- Current CPR (BLS) certification
- Board certified through AANP or ANCC
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
