Seeking a skilled Nurse Practitioner to conduct home health risk assessments in Dickenson County, VA, including areas such as Clintwood and Haysi. This position primarily involves visiting Medicare Advantage plan members in their homes to perform comprehensive risk assessments, which may encompass annual health reviews or post-discharge evaluations. Each assessment typically lasts about 45 minutes, allowing for in-depth interaction and a focus on holistic patient care.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to build your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage members
• Assess prior and current medical histories, conditions, and medication usage
• Educate patients about their existing medical conditions and prescriptions
• Identify gaps in care and offer recommendations to improve patient outcomes
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Virginia
• Current CPR certification (BLS)
• Board certified by AANP or ANCC
• Minimum six months of recent NP experience preferred
• MSN degree from an accredited nursing school

This role offers compensation of $80-$90 per evaluation, with additional stipends for mileage and administrative tasks.
