Job Description

Advance your career as a Nurse Practitioner specializing in home health by providing essential risk assessments for Medicare Advantage members throughout Wilkes County, NC. This per diem role offers a high degree of flexibility and autonomy, allowing you to create your own schedule and work shifts based on personal preferences and availability. Locations include North Wilkesboro, Wilkesboro, and Ronda, with the potential to cover any of these areas during your assigned shifts.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Self-scheduling to fit your needs
  • Choose your minimum weekly or bi-weekly commitment

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage beneficiaries in their homes
  • Evaluate medical history, current health conditions, and ongoing medication regimens
  • Educate patients about their diagnoses, health conditions, and prescribed medications
  • Assess for gaps in care and offer actionable recommendations to address them
  • No requirements for initiating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current BLS (CPR) certification
  • Board certified by AANP or ANCC
  • Minimum of six months' experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Compensation is competitive, with $80-$90 available per completed evaluation plus additional stipends for mileage and administrative responsibilities.
