Take your clinical expertise into the community as a Nurse Practitioner performing home health risk assessments. This per diem position is ideal for compassionate professionals seeking a flexible role that provides a meaningful impact on Medicare Advantage members throughout Niagara County, including North Tonawanda, Niagara Falls, and Lockport.
Schedule:
- Flexible shifts available any day Monday through Sunday
- Self-schedulingset your own hours
- Minimum weekly or bi-weekly hourly commitment
What Youll Do:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members in their residences
- Assess prior and ongoing medical history, medical conditions, and medication usage
- Offer education to patients on their health conditions and prescribed medications
- Detect any gaps in care and suggest actionable recommendations to resolve them
- Focus solely on assessment and education; treatment planning and prescribing medications are not required
Qualifications:
- Active Nurse Practitioner license in New York
- Successful completion of a MSN program from an accredited nursing institution
- Board Certification from AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- At least 6 months experience as a practicing Nurse Practitioner preferred, especially in a home health or similar clinical setting
