Job Description

Take your clinical expertise into the community as a Nurse Practitioner performing home health risk assessments. This per diem position is ideal for compassionate professionals seeking a flexible role that provides a meaningful impact on Medicare Advantage members throughout Niagara County, including North Tonawanda, Niagara Falls, and Lockport.

Schedule:

  • Flexible shifts available any day Monday through Sunday
  • Self-schedulingset your own hours
  • Minimum weekly or bi-weekly hourly commitment

What Youll Do:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members in their residences
  • Assess prior and ongoing medical history, medical conditions, and medication usage
  • Offer education to patients on their health conditions and prescribed medications
  • Detect any gaps in care and suggest actionable recommendations to resolve them
  • Focus solely on assessment and education; treatment planning and prescribing medications are not required

Qualifications:

  • Active Nurse Practitioner license in New York
  • Successful completion of a MSN program from an accredited nursing institution
  • Board Certification from AANP or ANCC
  • Current Basic Life Support (BLS/CPR) certification
  • At least 6 months experience as a practicing Nurse Practitioner preferred, especially in a home health or similar clinical setting
