Take the lead in advancing patient care by performing home health risk assessments for Medicare Advantage members across Lorain County, OH. As a Nurse Practitioner working on a per diem basis, you'll make a direct impact through focused, one-on-one patient interactions in a home setting. Schedule:

• Flexible shifts available Monday through Sunday
• Estimate and select your own schedule to match lifes demands
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members.
• Review and evaluate prior and current medical histories, existing medical conditions, and medication lists.
• Offer patient education on diagnoses and medication usage.
• Identify care gaps and recommend solutions to improve patient outcomes.
• No need to create treatment plans or prescribe medicationsfocus is on assessment and education.

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board Certification by AANP or ANCC
• MSN degree from an accredited program
• At least 6 months of NP experience preferred
