Deliver meaningful care to individuals in their homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers flexibility and the chance to make a direct impact on the health outcomes of Medicare Advantage plan members in the Columbia County area, including St. Helens, Scappoose, and Vernonia.

Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Self-scheduling; create a work calendar that fits your life
• Minimum weekly or bi-weekly hourly commitment to maintain continuity

Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage members
• Review and evaluate patients prior and current medical histories, medical conditions, and medication regimens
• Provide health education about diagnoses and prescribed medications to patients in a home setting
• Identify care gaps and offer practical recommendations to improve health outcomes
• Appointments may include annual assessments and post-discharge visits
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Valid Nurse Practitioner license in Oregon
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of experience as a practicing NP preferred
• Home health experience a plus
