Job Description

Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem position offers the flexibility to craft your own schedule, performing assessments in a variety of settings across Linn County, IA, including Cedar Rapids, Marion, and Hiawatha. Each visit typically takes about 45 minutes and may consist of either annual assessments or post-discharge check-ins, providing invaluable insight into patient needs and promoting continuity of care.

Schedule

  • Flexible shifts available any day, Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitments

Key Responsibilities

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and document prior and current medical history, conditions, and medications
  • Educate patients on their health conditions and prescribed medications
  • Detect care gaps and recommend actions for improved patient outcomes
  • No prescribing or treatment plan development required

Qualifications

  • Active Nurse Practitioner license in Iowa
  • Board Certification from AANP or ANCC
  • MSN from an accredited nursing program
  • Current CPR (BLS) certification
  • Minimum of 6 months recent experience as a Nurse Practitioner preferred
