Seeking a skilled Nurse Practitioner to provide home health risk assessments on a per diem basis throughout Linn County, IA. This dynamic opportunity allows you to work directly with Medicare Advantage plan members, supporting their health and well-being through comprehensive evaluations performed in the comfort of their homes. Home health risk assessments vary and may include annual wellness assessments or post-discharge visits, each typically lasting about 45 minutes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling options to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive health risk assessments for Medicare Advantage members in diverse locations within Linn County, including Cedar Rapids, Marion, and Hiawatha
- Evaluate patients prior/current medical history, medical conditions, and medication usage
- Educate patients on existing medical conditions and medication management
- Identify gaps in healthcare, providing actionable recommendations
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner license in Iowa
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months NP experience preferred
- MSN from an accredited nursing program
