Advance your career as a Nurse Practitioner by performing home health risk assessments throughout Lee County, FL. This per diem opportunity allows you to make a meaningful impact on Medicare Advantage members living in the Cape Coral, Fort Myers, and Lehigh Acres areas. As part of this role, you'll create your own schedule, providing much-needed flexibility in your professional life.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours according to your preferences
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage health plan members
• Evaluate patients past and current medical histories, conditions, and medication regimens
• Offer education to patients regarding their health conditions and medications
• Identify and document gaps in care, recommending appropriate next steps
• Visits typically last around 45 minutes and may include annual assessments or post-discharge evaluations
• No treatment planning or prescribing required

Qualifications:

• Active, unrestricted Nurse Practitioner license in Florida
• Board Certification through AANP or ANCC
• Graduate of an accredited MSN program
• Current Basic Life Support (BLS/CPR) certification
• Six months or more of recent Nurse Practitioner experience preferred

You will receive $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
