Take your nurse practitioner experience into the community with a rewarding per diem role conducting home health risk assessments. Schedule:

• Flexible shifts available seven days a week
• Ability to design your own schedule
• Minimum commitment requirements on a weekly or bi-weekly basis

Key Responsibilities:

• Complete comprehensive home health risk assessments for Medicare Advantage plan members
• Evaluate and document prior/current medical history, active medical conditions, and medication usage
• Provide patient education regarding health management, medications, and chronic conditions
• Identify healthcare gaps and offer evidence-based recommendations to improve outcomes
• Focus exclusively on assessmentno treatment planning or prescribing is required

Qualifications:

• Active Nurse Practitioner license in Pennsylvania
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing school
• At least 6 months of current NP experience preferred

You will complete each assessment in approximately 45 minutes, with visit types spanning annual health assessments and post-discharge evaluations. Scheduling is location-dependent; visits may occur in Erie, Millcreek, Corry, and other parts of Erie County.

This per diem opportunity provides competitive earnings, with $80$90 per evaluation plus mileage reimbursement and compensation for administrative time.
