Advance your nursing career with per diem opportunities conducting home health risk assessments for Medicare Advantage plan members across Stark County, Ohio. Schedule:
- Flexible shifts are available Monday through Sunday
- Create and manage your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients prior and current medical histories, medical conditions, and medication usage
- Deliver clear education to patients regarding their medical conditions and medications
- Detect care gaps and provide actionable recommendations for care coordination
- Conduct all assessments in patients homes within areas such as North Canton, Massillon, Alliance, and greater Stark County
- Focus solely on assessmentsno treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- National board certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of recent NP experience preferred
This position offers a competitive compensation structure of $80$90 per evaluation, alongside a mileage stipend and administrative time reimbursement.
