Deliver essential care in a home health setting by performing comprehensive risk assessments for Medicare Advantage members across Warren County. As a Nurse Practitioner specializing in home health, you will be the crucial link between patients and their care plans, ensuring each individual receives a thorough evaluation in the comfort of their own home. This per diem opportunity empowers you to manage your own schedule, allowing maximum flexibility and work-life balance.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: create your own hours
- Satisfy a minimum hourly commitment weekly or bi-weekly
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Travel to patient residences in Norlina, Warrenton, Macon, and other Warren County locations
- Assess medical history, current conditions, and medication usage
- Educate individuals and caregivers on diagnoses and medication management
- Identify care gaps and make recommendations to optimize patient outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Up-to-date CPR certification (BLS)
- Board Certified through AANP or ANCC
- Minimum of 6 months of NP experience (preferred)
- MSN from an accredited school of nursing
This rewarding opportunity includes compensation of $80$90 for each evaluation performed, as well as a mileage stipend and compensation for necessary administrative time.
