Deliver exceptional patient-centered care while enjoying the flexibility of a per diem role as a Nurse Practitioner specializing in home health risk assessments. This position focuses on conducting assessments for Medicare Advantage plan members residing in Norlina and the broader Warren County area, including Warrenton and Macon.
Schedule:
- Flexible shifts available Monday through Sunday

- Options for minimum weekly or bi-weekly commitments
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review each patients prior and current medical, surgical, and medication history
- Offer education regarding ongoing health conditions and appropriate medication use
- Recognize gaps in care, identify health risks, and recommend appropriate follow-ups or interventions
- No treatment plan development or prescription orders required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Master of Science in Nursing (MSN) from an accredited institution
- Board Certification through AANP or ANCC
- Valid CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience (preferred)
