Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Minimum hourly commitment required weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Assess patients prior and current medical history, health conditions, and medication use
- Educate members about their medical conditions and medication regimens
- Identify gaps in care and suggest recommendations to enhance care continuity
- No prescribing or treatment plan creation required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months' recent NP experience preferred
- Master of Science in Nursing from an accredited program
This position offers competitive compensation of $80-$90 per completed evaluation, along with a mileage stipend and payment for certain administrative tasks.
