Advance your expertise as a Nurse Practitioner specializing in home health by conducting comprehensive risk assessments for Medicare Advantage plan members in the Norfolk area. This per diem opportunity offers unparalleled scheduling flexibility, allowing you to tailor your shifts between Monday and Sunday and set your own work hours. With a minimum weekly or bi-weekly hourly commitment, you can seamlessly integrate this role into your current routine, all while making a meaningful impact through at-home patient evaluations.
Responsibilities include:
- Conducting annual home health risk assessments in patient residences.
- Completing evaluations averaging 45 minutes per visit, including annual health reviews and post-discharge follow-ups.
- Assessing patients prior and current medical history, conditions, and medication use.
- Providing clear education about medical conditions and medication adherence.
- Identifying gaps in care and suggesting evidence-based solutions.
- Focusing solely on assessment without responsibility for devising treatment plans or prescribing medications.
Qualifications:
- Active Nurse Practitioner License in Virginia.
- Board Certification from AANP or ANCC.
- MSN degree from an accredited nursing program.
- Current CPR certification (BLS).
- Minimum of 6 months recent NP experience preferred.
Compensation for this role is $80-$90 per evaluation, with an additional mileage stipend and administrative time included.
