Job Description

Take your Nurse Practitioner expertise into the community and make a meaningful difference as you perform independent home health risk assessments. These in-person visits offer the unique opportunity to serve Medicare Advantage members throughout St. Lawrence County, NYincluding areas such as Potsdam, Massena, Ogdensburg, and Canton. As a practitioner in this per diem role, youll conduct health evaluations, empower members through education, and play a key role in identifying health needsall while enjoying true schedule flexibility.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to create and manage your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive in-home health risk assessments for Medicare Advantage plan members
  • Evaluate medical, medication, and health history, integrating findings into reports
  • Deliver education on medical conditions and current medications to patients and caregivers
  • Identify care gaps and provide recommendations to help close them
  • Visits may include annual health assessments or post-discharge check-ins
  • Responsible for assessments onlyno prescribing or new treatment plans required

Qualifications:

  • Active Nurse Practitioner License in the State of New York
  • Board Certification via AANP or ANCC
  • Current BLS (CPR) certification
  • Minimum of 6 months current Nurse Practitioner experience is preferred
  • MSN degree from an accredited school of nursing

This role offers a competitive evaluation-based pay structure of $80$90 per completed assessment, with additional compensation for mileage and administrative time.
