Make a meaningful impact by delivering compassionate, personalized healthcare directly to patients in their homes. As a Nurse Practitioner specializing in home health, you'll conduct comprehensive risk assessments for Medicare Advantage plan members across St. Lawrence County, NY. Each home health evaluation lasts about 45 minutes and may consist of an annual health review or a post-discharge visit, tailored to the members unique needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform in-home annual risk assessments for Medicare Advantage health plan members
- Review previous and current medical history, conditions, and medication records
- Educate patients on medical diagnoses and medication usage
- Identify gaps in care and recommend resolution measures
- No requirement for treatment plan development or medication prescribing
Qualifications:
- Current Nurse Practitioner license in New York
- Board Certification via AANP or ANCC
- Active CPR certification (BLS)
- Graduation from an accredited MSN program
- At least six months recent experience as a Nurse Practitioner is preferred
