Job Description

Deliver high-quality care in a dynamic, patient-centered home health role serving Medicare Advantage plan members across Santa Cruz County, AZ, including the areas of Rio Rico, Tubac, and Sonoita. As a Nurse Practitioner, youll conduct home-based health risk assessments, contributing to preventive care and improved health outcomes for a diverse patient population.

Schedule:

  • Flexible shifts available throughout the week, Monday through Sunday
  • Control your own schedule to fit your personal and professional commitments
  • Minimum weekly or bi-weekly hour commitment based on your availability

Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate patients medical histories, current health conditions, and medication regimens
  • Educate members about health conditions and medications, supporting patient understanding and engagement
  • Identify care gaps and provide recommendations, promoting continuity and quality of care
  • Each assessment visit averages 45 minutes; activities may include annual assessments or post-discharge follow-ups
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Current Nurse Practitioner license in Arizona
  • Board certification through AANP or ANCC
  • Valid CPR/BLS certification
  • Minimum of six months experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited institution
