Enjoy the flexibility of per diem work as an experienced Nurse Practitioner specializing in home health risk assessments. In this dynamic role, you'll perform focused evaluations for Medicare Advantage members in Hamilton County, including areas such as Carmel, Fishers, and Noblesville. Each assessment averages 45 minutes and may range from annual health check-ins to important post-discharge follow-ups, ensuring members receive comprehensive care in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Set a preferred weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Review previous and current medical history, conditions, and medications
- Provide individualized education regarding health conditions and medications
- Identify gaps in care and make appropriate recommendations
- No requirement for treatment plan development or medication prescribing
Qualifications:
- Active Nurse Practitioner license in Indiana (IN)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR (BLS) certification
- Minimum of six months current NP experience preferred
