Make a meaningful difference in patient care by performing in-home health risk assessments for Medicare Advantage plan members throughout Dillon County, SC. As a Nurse Practitioner in this per diem opportunity, youll work autonomously, helping vulnerable populations manage their health while enjoying flexible scheduling tailored to your availability.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create your own workdays
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual risk assessments in members homes across Dillon, Latta, Lake View, and surrounding areas
• Review patients prior and current medical history, assess chronic conditions, and update medication profiles
• Educate individuals on disease management, prevention, and proper medication usage
• Detect gaps in care and provide actionable recommendations to close them
• Focus on assessment and education onlyno medication prescribing or treatment planning is required
• Time per assessment averages 45 minutes

Qualifications:

• Unrestricted Nurse Practitioner license in South Carolina
• Board certification from AANP or ANCC
• Current CPR certification (BLS required)
• MSN degree from an accredited school of nursing
• At least 6 months of current NP experience preferred
