Take on a valued clinical role providing in-home health risk assessments for Medicare Advantage members across Buffalo and Erie County, NY. This per diem opportunity allows you to deliver comprehensive care in a home setting, making a direct impact on patient outcomes and experience.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Commit to weekly or bi-weekly hours as fits your availability

Key Responsibilities:

• Conduct in-depth, in-home health risk assessmentsapproximately 45 minutes per visitfor Medicare Advantage plan participants
• Gather and evaluate medical histories, current health conditions, and medication use
• Provide patient education on disease management, preventative care, and safe medication practices
• Identify care gaps and recommend appropriate actions for follow-up or coordination
• Focus exclusively on assessments; no ongoing care, treatment planning, or prescribing involved

Qualifications:

• Active Nurse Practitioner license in the state of NY
• Board Certified through AANP or ANCC
• Current BLS CPR certification
• Master of Science in Nursing (MSN) from an accredited program
• Minimum of 6 months recent Nurse Practitioner experience preferred
• Strong interpersonal and time management skills
