Deliver high-quality patient assessments in a dynamic home health environment as an experienced Nurse Practitioner. This per diem opportunity focuses on providing comprehensive home health risk assessments for Medicare Advantage members throughout Niagara County, NY, including Niagara Falls, Lockport, and North Tonawanda. Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual home health risk assessments for plan members, with each visit averaging approximately 45 minutes
• Evaluate patients prior and current medical history, ongoing medical conditions, and medication use
• Educate members on managing their medical conditions and proper medication usage
• Identify any gaps in care and offer clear recommendations for follow-up
• Visits may include annual assessments or post-discharge evaluations
• No diagnosis, creation of treatment plans, or prescribing required

Qualifications:

• Active Nurse Practitioner license in New York State
• MSN degree from an accredited nursing program
• Current Board Certification through AANP or ANCC
• Current BLS/CPR certification
• At least 6 months of recent NP experience preferred

This per diem position offers $80$90 for each completed evaluation, with an additional mileage stipend and compensation for administrative time. You'll experience meaningful patient interactions while maintaining maximum schedule flexibility and a strong support network.
