Embrace a rewarding per diem opportunity working as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members in Niagara County, NY. Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours
- Choose your minimum weekly or bi-weekly commitment
Key Responsibilities:
- Perform annual home health risk assessments in members' homes
- Assess medical history, current conditions, and medication usage
- Educate patients on their health conditions and treatments
- Identify gaps in care and recommend solutions for follow-up
- Focus solely on evaluationno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing program
- Current CPR certification (BLS)
- At least six months of recent experience as a Nurse Practitioner preferred
