Dedicated Nurse Practitioners with a passion for patient-centered care are invited to provide home health risk assessments for Medicare Advantage members in Jasper County and surrounding areas. This per diem opportunity offers exceptional flexibility, allowing you to create your own schedule and maintain a healthy work-life balance.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform thorough annual home health risk assessments in patients homes
- Assess medical histories, current conditions, and medication use
- Provide clear education to patients about health conditions and prescribed medications
- Identify potential care gaps, offering recommendations to address or resolve them
- Visits may involve annual assessments, post-discharge evaluations, or follow-up care
- No creation of treatment plans or prescription responsibilities
Qualifications:
- Current Nurse Practitioner license in Texas
- Valid CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing from an accredited institution
In this role, each evaluation is compensated at $80$90, inclusive of a mileage stipend and recognition for administrative time, rewarding your expertise and dedication.
