Embrace a dynamic per diem opportunity designed for experienced Nurse Practitioners skilled in home health care. This role focuses on performing home health risk assessments for Medicare Advantage plan members throughout Lincoln County, including areas such as Iron Station, Lowesville, and Westport. Each assessment typically takes about 45 minutes and may include annual health evaluations or post-discharge visits, offering a varied and meaningful patient care experience.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingcreate your preferred work schedule
- Choose between weekly or bi-weekly minimum hour commitments
Key Responsibilities:
- Perform thorough home health risk assessments for Medicare Advantage health plan members
- Review and evaluate both prior and current medical history, health conditions, and medications
- Educate members about their health conditions and prescribed medications
- Identify care gaps and make recommendations to enhance care coordination
- Prescription management and treatment plan development are not part of this role
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board Certification through AANP or ANCC required
- MSN degree from an accredited institution
- At least 6 months of recent Nurse Practitioner experience preferred
