Advance your nurse practitioner career with rewarding per diem work in home health risk assessments. As an NP specializing in home health, you will visit Medicare Advantage plan members throughout Cocke County, conducting focused evaluations in Newport, Parrottsville, and nearby areas. Each assessment typically takes about 45 minutes and may vary between annual health reviews and post-discharge visits.

Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate previous and current medical history, conditions, and medication use
- Educate patients about their medications and ongoing medical conditions
- Identify care gaps and offer recommendations to address them
- Note: You will not be responsible for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN degree from an accredited school of nursing
- At least 6 months of NP experience preferred
