This per diem Nurse Practitioner role involves conducting one-on-one home visits to Medicare Advantage plan members. Youll play a vital part in preventative care while enjoying flexibility and autonomy in your work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling optionsbuild your own workweek
- Minimum hourly commitment can be weekly or bi-weekly
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage health plan members
- Review and evaluate prior/current medical history and conditions
- Assess medication history and provide related education
- Identify potential gaps in care, offering relevant recommendations
- Deliver patient education on health management and medication usage
- No responsibility for prescribing medications or developing treatment plans
Required Qualifications:
- Active Nurse Practitioner license in Virginia (VA)
- Board Certification by AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience is preferred
