Job Description
Drive patient care outcomes with a dynamic role that allows you to deliver meaningful home health risk assessments to Medicare Advantage plan members in the Hampton area. As a Nurse Practitioner specializing in home health, youll complete comprehensive evaluations in patients homes, empowering them with insight and recommendations to enhance their well-being.

Responsibilities include:
- Conducting annual home health risk assessments, including both annual and post-discharge visits
- Thoroughly evaluating patient medical and medication histories, prior and current conditions
- Educating patients about their existing health conditions and how to manage their medications effectively
- Identifying any immediate gaps in care and offering practical recommendations to close those gaps
- Focusing on clinical assessment without responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Virginia
- AANP or ANCC Board Certification
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner is preferred
