Healthcare facilities are seeking skilled Nurse Practitioners to perform home health risk assessments for Medicare Advantage plan members across Carteret County, NC. These per diem opportunities offer flexible scheduling and autonomy, with work settings in Morehead City, Beaufort, Newport, and other nearby locations. Assignments will be matched to your preferred availability, allowing you to build your own schedule with minimum weekly or bi-weekly commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hour commitment required
Responsibilities:
- Conduct comprehensive home health risk assessments, typically lasting around 45 minutes
- Assess patients prior and current medical and medication histories
- Educate members on health conditions, medications, and preventive care
- Identify gaps in care, recommend solutions, and document findings
- Perform annual health evaluations and post-discharge visits as assigned
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Six months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
This opportunity provides compensation of $80$90 per completed evaluation, along with a mileage stipend and administrative time for documentation.
