Job Description

Deliver quality patient care in the home setting by performing clinical risk assessments for Medicare Advantage plan members throughout Jackson County, AR. This per diem opportunity is ideal for Nurse Practitioners seeking flexible scheduling and the chance to make a direct impact on community health. The position covers a variety of locations, including Newport, Tuckerman, and Diaz, and allows providers to create their own schedule with a minimum weekly or bi-weekly commitment.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling with the ability to set your own hours
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete comprehensive home health risk assessments, including annual health reviews and post-discharge visits
  • Assess patient history, medical conditions, and medication regimens
  • Educate patients about their medical conditions and prescribed medications
  • Identify care gaps and provide actionable recommendations
  • No direct treatment plan creation or prescribing involved

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS required)
  • Board Certified through AANP or ANCC
  • Minimum of 6 months experience as a Nurse Practitioner preferred
  • MSN degree from an accredited program
