This per diem role offers autonomy, variety, and the opportunity to shape your own work schedule while providing essential assessments in areas including Newport, Tuckerman, and Diaz.
Schedule
- Flexible shifts available Monday through Sunday
- Design your own schedule to accommodate personal commitments
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Assess prior and current medical history, chronic conditions, and medication use
- Deliver education to members regarding their health conditions and prescribed medications
- Identify gaps in care and provide recommendations to help close these gaps
- No treatment plan creation or medication prescribing requiredfocus is on holistic assessment and education
Qualifications
- Active Nurse Practitioner license in Arkansas
- Current BLS CPR certification
- Board Certification via AANP or ANCC
- MSN degree from an accredited school of nursing
- At least 6 months of current Nurse Practitioner experience preferred
