Job Description

Bring your expertise as a Nurse Practitioner specializing in Home Health to support Medicare Advantage plan members through in-home risk assessments. This per diem position offers the flexibility to design your own work schedule, with shifts available any day of the week. Youll have the opportunity to travel within scenic Avery County, NC, conducting visits in areas including Banner Elk, Beech Mountain, Crossnore, Elk Park, and nearby communities. Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your preferred working days and hours
  • Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform thorough home health risk assessments (each approximately 45 minutes)
  • Evaluate patients prior/current medical history, conditions, and medications
  • Educate members about health conditions and prescribed medications
  • Identify and report any gaps in care, recommending practical solutions
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of experience as a practicing NP is preferred

Compensation includes $80-$90 for each completed evaluation, along with additional mileage stipend and paid administrative time, ensuring your time and effort are valued.
