An exciting per diem opportunity is available for skilled Nurse Practitioners with an interest in home health risk assessments. This role involves traveling within Kay County, including communities such as Newkirk, Blackwell, Tonkawa, and Ponca City, to conduct patient evaluations in the comfort of their homes. The primary focus will be on Medicare Advantage plan members, performing visits that may include annual health assessments or post-discharge evaluations. Each assessment typically lasts about 45 minutes, encompassing a thorough review of the members medical history, current conditions, and medications.

Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage health plan members
- Review and document prior and current medical history, underlying conditions, and medication usage
- Offer educational guidance on disease management and proper use of medications
- Identify and document gaps in care while providing recommendations for follow-up
- No prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner License (OK)
- Board certified (AANP or ANCC)
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- Minimum of 6 months recent NP experience (preferred)
In addition to a rewarding clinical experience and the autonomy to create your own schedule, this position offers compensation of $80-$90 per evaluation, with a mileage stipend and additional pay for administrative time.
