Job Description

Deliver vital healthcare services while enjoying the autonomy and flexibility of per diem work as a Nurse Practitioner specializing in home health risk assessments. This role focuses on evaluating Medicare Advantage plan members throughout Kay County, including Blackwell, Newkirk, Tonkawa, and Ponca City. Candidates should be prepared to travel locally within these areas and may be scheduled at any location during a shift.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule to fit your lifestyle
  • Minimal weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Review and document prior/current medical history, medical conditions, and medication usage
  • Educate patients regarding their health conditions and medication regimens
  • Identify gaps in care and provide actionable recommendations
  • Focus on assessment and educationno need to create treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner License in Oklahoma
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing school
  • A minimum of six months recent Nurse Practitioner experience strongly preferred
