Flexible, per-diem opportunity available for an experienced Nurse Practitioner to conduct home health risk assessments in the Newcomerstown area, supporting Medicare Advantage plan members throughout Guernsey County. This role provides the chance to deliver high-impact care to a diverse population in settings ranging from annual evaluations to post-discharge follow-ups, with each assessment typically lasting about 45 minutes. Locations may include Cambridge Township, Cambridge, and Jackson Township, with flexible shift assignments based on your preferred schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: create your own schedule
- Minimum hourly commitment per week or bi-weekly period
Key Responsibilities:
- Complete annual in-home health risk assessments for Medicare Advantage members
- Assess recent and current medical histories, conditions, and medication usage
- Educate patients regarding their health conditions and medications
- Identify care gaps and make recommendations for follow-up care
- No requirements for treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner (NP) license in Ohio
- Board Certification through AANP or ANCC required
- Current CPR certification (BLS)
- MSN degree from an accredited school of nursing
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation includes $80-$90 per completed evaluation, along with a mileage stipend and additional compensation for administrative time.
