Step into an impactful per diem role performing home health risk assessments as a Nurse Practitioner in Dutchess County, NY. This position offers outstanding flexibility, allowing you to create your own schedule with opportunities to work shifts between Monday and Sunday. You will visit Medicare Advantage plan members in their homes across locations like Poughkeepsie, Beacon, Fishkill, Rhinebeck, and beyond, conducting thorough evaluations that typically take about 45 minutes.

Key Responsibilities:
- Perform annual health risk assessments and post-discharge visits in home settings
- Review and evaluate past and current medical history and medication usage
- Provide education to patients regarding their health conditions and medications
- Detect gaps in care and make recommendations to improve care quality
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner (NP) license in New York
- CPR certification (BLS) required
- Board Certification via AANP or ANCC
- At least 6 months of recent nurse practitioner experience preferred
- MSN degree from an accredited nursing program
This role is compensated at $80$90 per completed evaluation, with additional mileage stipend and administrative time covered, reflecting your clinical expertise and dedication to quality healthcare delivery.
