Job Description

Make a meaningful difference in patient care by performing home health risk assessments throughout Dutchess County, NY. This per diem opportunity allows nurse practitioners to utilize their clinical expertise, offering flexible shifts and autonomy in scheduling. Assessments may take place in a variety of locations, including Poughkeepsie, Beacon, Fishkill, and Rhinebeck, and typically last about 45 minutes. Depending on the visit, you may conduct an annual health assessment or visit post-discharge patientsprimarily for Medicare Advantage plan members.

Schedule:

  • Flexible shifts available from Monday to Sunday
  • Self-schedulingcreate your own work hours
  • Minimum weekly or bi-weekly hourly commitment as required

Core Responsibilities:

  • Conduct home health risk assessments, reviewing each patient's medical and medication history
  • Evaluate and document current and prior health conditions
  • Educate members about chronic conditions, medications, and healthy lifestyle practices
  • Identify any gaps in care and recommend strategies to address them
  • No requirements for formal treatment planning or medication prescribing

Desired Qualifications:

  • Active Nurse Practitioner license in NY
  • Current CPR (BLS) certification
  • Board certified by AANP or ANCC
  • Minimum of six months experience as a Nurse Practitioner (preferred)
  • MSN degree from an accredited nursing program
