Seeking a skilled healthcare provider for per diem work conducting comprehensive home health risk assessments. This flexible position is ideal for a Nurse Practitioner who values both autonomy and meaningful patient interactions. Candidates will serve Medicare Advantage plan members across various communities in Vanderburgh County and surrounding areas.
Schedule:
- Flexible shifts available, MondaySunday
- Ability to create your own schedule
- Options for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual home health risk assessments in patient homes
- Review prior and current medical history, conditions, and medication usage
- Educate patients on their medical conditions and medication regimens
- Identify gaps in care and offer practical recommendations to improve outcomes
- Focus exclusively on assessment and educationno treatment plan creation or prescribing required
Qualifications:
- Active Nurse Practitioner license in IN
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience is preferred
Enjoy compensation of $80$90 for each completed evaluation, including a mileage stipend and administrative time, making this a rewarding role both financially and professionally.
