This per diem opportunity is ideal for Nurse Practitioners experienced in home health eager to provide meaningful care on a flexible schedule. Deliver vital home health risk assessments to Medicare Advantage plan members across Alachua County, including Gainesville, Alachua, and Newberry. Home health visits typically last around 45 minutes and may include annual wellness checks or post-discharge evaluations. With the ability to create your own schedule, you can balance your professional commitments alongside personal priorities.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct in-home annual health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, existing conditions, and medication regimens
- Educate patients on their medical conditions and proper medication usage
- Identify care gaps and provide actionable recommendations for follow-up
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Current, unrestricted Nurse Practitioner license in Florida
- Board certification through AANP or ANCC
- Valid CPR certification (BLS)
- MSN from an accredited nursing program
- At least 6 months of recent NP experience preferred
This position offers compensation of $80-$90 per evaluation, along with a mileage stipend and administrative time, rewarding you for your expertise and commitment to patient care.
