Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct thorough home health risk assessments in member residences, each visit averaging about 45 minutes
- Evaluate patient medical histories (prior and current), conditions, and medication use
- Educate patients on their medical conditions and appropriate medication management
- Identify gaps in care and provide actionable recommendations
- Focus exclusively on assessmentsno requirement to prescribe or create treatment plans
Qualifications:
- Active Nurse Practitioner (NP) license in Tennessee
- Current BLS/CPR certification
- Board certification via AANP or ANCC
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
