Experienced Nurse Practitioners are needed for flexible per diem assignments performing home health risk assessments within Yamhill County. This role focuses on completing comprehensive health evaluations for Medicare Advantage plan members in their homeshelping to bridge gaps in patient care and ensure continuity of services.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual home health risk assessments in members homes throughout Newberg, Dundee, McMinnville, and surrounding areas
• Review each patient's prior and current medical history, existing medical conditions, and full medication profile
• Educate patients on their health conditions and prescribed medications
• Identify potential gaps in care and offer recommendations to support better patient outcomes
• No development of treatment plans or prescribing of medications is involved in this position

Qualifications:

• Valid Nurse Practitioner license in the state of Oregon
• Active CPR certification (BLS)
• Board Certification via the AANP or ANCC
• 6 months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program

This per diem opportunity offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and additional compensation for administrative time.
